I Have an TeleTherapy Appointment: Dr. Stein Accepts My Health Insurance Plan
If the following applies to you, please use the health insurance co-payment button below prior to the TeleTherapy (video) appointment you personally scheduled with Dr. Stein:
1. I have contacted Dr. Stein directly and we have a TeleTherapy appointment set.
2. I have health insurance and Dr. Stein confirmed he can take my insurance. I have been directed to pay my insurance co-payment prior to my scheduled appointment.
3. I understand that if I cancel the appointment at least 24 hours in advance I will have my co-payment refunded.
4. If I cancel my appointment without providing at least 24 hours notice or fail to show up for the appointment, my co-payment cannot be refunded.
