Appointment, Fee and Cancellation Policies

  1.  Unfortunately, no insurance is accepted at this time.  However, we have forms for most insurance companies that will allow you to request reimbursement from out of network providers.
  2. Billing information is required at the time you make an appointment. Your account will be charged only at the time of service.
  3. No-Shows for appointments (failing to cancel in advance of scheduled appointment) will be billed at 50% of the standard fee.
