Appointment, Fee and Cancellation Policies
- Unfortunately, no insurance is accepted at this time. However, we have forms for most insurance companies that will allow you to request reimbursement from out of network providers.
- Billing information is required at the time you make an appointment. Your account will be charged only at the time of service.
- No-Shows for appointments (failing to cancel in advance of scheduled appointment) will be billed at 50% of the standard fee.
